Documents Required for School Entry

In order for your child to enroll in Mayfield Elementary School, Kentucky State law requires that you provide the following:

1. A copy of your child’s official birth certificate
2. A current immunization certificate
3. Proof of a medical exam on the proper form issued no more than one year prior to initial entry
4. If you child is less than seven years old, a dental screening on the proper Kentucky form
5. Proof of an eye exam completed by an optometrist or ophthalmologist on the proper form
